Responsibilities
The responsibilities of the Home Care Supervisor LPN include:
Functions as a team supervisor, responsible for processing all assigned incoming referrals for all Meridian At Home product lines (Home Health, Infusion, Hospice, Private Pay and Technology) by utilizing the team model.
The Home Care Specialist Supervisor LPN will work under the supervision of a RN and will be responsible for assessing patient eligibility for the specific program by performing a review of all clinical and insurance information, obtains necessary documentation to support clean billing, requests completed face to face document (if not yet obtained), and assures adherence to regulatory requirements.
Functions as a clinical resource for the team and is responsible for distributing work evenly among team members. Accountable for clinically complete referrals initiated by team members and process all assigned referral efficiently and accurately.
Ensure open communication to operations. Assures team meets 95% audit accuracy for assigned referrals.
Liaison between MD offices and external facilities demonstrating customer service skills while assuring clinical documentation to support home care referrals, under the supervision of a RN. Responsible to maintain all assigned reports.
Identifies opportunities to further assist customer needs with Meridian At Home services and continually supports growth of all product lines.
Qualifications
Current NJ LPN license required
1 or more years of LPN experience in an acute care setting
Knowledge of home care preferred
Strong organizational, interpersonal, written and verbal communication skills
Basic computer knowledge/skills



Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
