Palliative Care Svc Associate

Palliative Care Associate coordinates consultations, phone touches and mailings. Responsibilities also include managing the back-office operations of physician engagement. Process Improvement, develop and managing the clinician/patient interactions. Implement, manage and streamline scheduling system (in web chart, Care Journey's data management system) to needs of the clinicians, patients, CMS and MH reporting requirements.
Back office physician engagement: Clinician engagement documentation completed on a timely basis, authorization forms sent and processed as per policy.
Clinical Scheduling system: Clinician (MD, APN, RN, SW, Chaplain) and patients scheduled as per acuity level on a continuing basis.
Continual process improvement: Streamline of administrative, patient, and clinical scheduling and coordination.
Coordinates In-Service and Marketing needs: Assists in Palliative Care in-service needs as well as marketing activities.
Customer Relations: Establishes and maintains rapport and mutual respect with co-workers, public and key contact persons through oral and written means to foster optimum working environment.
Customer Satisfaction: Actively engages customers in a warm and friendly way in an effort to make their day.
Data Entry Accuracy: Initiates and accurately completes assigned standard non-standard reports up to the Director level within defined time frame (e.g. meeting minutes, data requests, record reviews, dictated and composed correspondence). Completes business documentation pertaining to department according to task list and requests (e.g. monthly reports, budget updates, patient accounting statistics, utilization reviews).
Information Management: Establishes and oversees maintenance of record keeping system, ensuring retrieval of information in an accurate, efficient manner (e.g. administrative records, medical records, membership records).
Office Skills: Copy, collate, file, and maintain data accurately as per departmental functions and requests. Prepares necessary reports. Reports are prepared and sent by required deadlines. Enrollment: Manages Physician and patient information processes and documentation.
Policy Development: Develops policies as necessary regarding scheduling and back ?office physician engagement Policies kept up to date.
Proficiency in Excel, Word, Outlook, and other Microsoft products. Excellent phone and interpersonal skills. Proactive, detail-oriented, and skilled in efficiency and process improvement. Team player. Scheduling experience preferred.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
